Consent Form for Photography, Activities, and Medication Administration

• Photography Consent 

I am, hereby authorise my child/children, to have their picture(s) taken during play at Monkeybars Playcentre and MonkeyBars day camp. Said pictures may be used as promotional material on social media (FB, Instagram, Twitter) and our websites. They will never be used or given out to third parties and will be treated as the property of Monkeybars Entertainment LTD.

•  WAIVER AND RELEASE FORM 

To participate in Activities, and/or other Events of the Company during the duration of the admitted time and allow participation in all events physically offered on the Premises of the “company”.

• IMPORTANT INFORMATION: 

By signing this Activity and Release Form, I acknowledge and agree that in consideration services provided by the Company, of payment for the Activity, and of permitting the Child(ren) to participate in the Activity, the sufficiency of which is acknowledged, I forever release and discharge the Company (including but not limited to its agents, owners, officers, directors, assigns, legal representatives, employees, and successors) from all manner of actions, causes of action, debts, accounts, bonds, contracts, claims and demands for or by reason of any injury to person or property, including injury resulting in the death of the Child(ren), which has been or may be sustained as a consequence of the Child(ren)’s participation in the Activity, and notwithstanding that such damage, loss or injury may have been caused solely or partly by the negligence of the Company.

I acknowledge that the Child(ren) listed above will not be permitted to participate in the Activity unless I accept, acknowledge, and sign this Activity and Release Form; and I further understand that this Activity and Release Form is provided with the express intention of effecting the extinguishment of certain legal obligations, rights, and duties owed to the Child(ren) and myself, with the specific intention of binding the Child(ren) and myself as well as any subsequent legal guardians, executors, administrators, legal representatives, or assigns of myself or the Child(ren).

I acknowledge that I have provided any and all information to the Company regarding any and all ailments, limitations, disabilities, or diagnoses of the Child(ren) that would affect their ability to interact with other children or the Activity.

I acknowledge that I have read and understand this Activity and Release Form, the posted “Rules of Play” stated by the play centre, and the risks associated with participation in the facilities offerings. By signing this Activity and Release Form, I agree to be forever prevented from suing or otherwise claiming against the Company for any property loss or personal injury that I or the above Child(ren) may sustain while participating in the Activity.

• Medication Administration Release Form 

This form is to be completed in full by Parents/Guardians for all Day Camp Participants and applies any time medication is to be administered by Day Camp personnel to a child while in Monkeybars playcentre supervision.

In signing this form, the parent or legal guardian releases MonkeyBars Playcentre, its servants, employees, and agents from and against all claims, suits, demands, and actions whatsoever taken now or in the future, which may arise by reason of the administration of medication to the child. The action taken by staff as requested above is both requested and authorized. MonkeyBars Staff are authorized to take emergency action as deemed appropriate.
